Analysing the past, planning the future, for the hazard of management
One may readily accept that the hazards of work situations are related to the management, and the management systems, which supervise and control those work situations. This author has anecdotal data which show that 'everyone knows' there is a relationship between those two factors, work hazards and management, so although the knowledge exists., in practice, in industry, many ways are found to avoid making the connection. The author's research into the chemical industry identified the connection, an identity which has been strengthened by many accident investigations performed through the last ten years and by a serious incident in Australia. This paper presents the author's research to show the connection between hazards and management, uses accident investigations to confirm that connection, and uses a brief description of the incident further to demonstrate the connection.
