Unplanned shutdowns plus lack of knowledge equals incidents
Two incidents and a near miss, each precipitated by an unplanned shutdown, are reviewed. In every case, a lack of understanding of the process control and/or chemistry led to an uncontrolled reaction that resulted in process equipment damage and, in one case, injured several people. The incidents are described in detail, and some general lessons and recommendations are drawn from them. (C) 2004 American Institute of Chemical Engineers Process.
